You have 14 days from the day of purchase to return the product you are dissatisfied with. Before completing this form please contact Ulmer Bros.
at 1-866-518-9096 to receive clearance. After this return authorization form has been received by Ulmer Bros and you have been given proper approval, you will be issued a credit for the item on your charge card that is on file for the purchase.
Refunds will not be given for shipping/handling charges.
The product must be in
Re-saleable /original condition with all packaging in order to receive your refund. All items returned are subject to a 25% restocking fee. Your credit card will only be credited for the returned item only. Customer is responsible for return shipping cost. If the customer is in agreement to the above terms please fill the bottom portion out.
